This registration form and the assigned Personal Identification Number (PIN) are to be used with the IVR system and only by Police, Law Enforcement, and Public Safety agencies as defined by the Department of Justice.

PINs can be shared. If your organization has been assigned a PIN already, please use that PIN. If no PIN has been assigned to your organization, please submit only one registration form to request a PIN for your organization’s use. If it is necessary that there be a specific PIN assignment for each individual in your organization, please submit a separate registration form for each individual who requires a unique PIN.
